Scope of Warranty
- Applies to GDT Supply products purchased through authorized channels and used as intended per the manufacturer’s Instructions for Use (IFU).
- Covers defects in materials or workmanship and products that do not meet stated quality/performance standards.
- Remedy is replacement of the affected product at no additional charge.
Exclusions
- Immediate restorations or provisional components.
- Consumables such as inserts and silicone caps.
- Damage due to physical trauma, accidents, misuse, modification, or medical malpractice.
- Failures related to contraindications (e.g., alcoholism, diabetes, smoking, drug addiction) or inadequate oral hygiene.
- Consequential costs (e.g., laboratory fees, clinical or dental treatment expenses).
Eligibility Conditions
- Products must be used with proper surgical/prosthetic techniques and compatible tools per IFU.
- Patient maintenance consistent with accepted oral-hygiene standards.
- Return of the failed product and related components for evaluation within 60 days of the event.
- Include relevant documentation (e.g., pre- and post-operative radiographs for implant claims).
Claim Procedure
- Contact GDT Supply USA Customer Support.
- Request and complete the Product Replacement Form.
- Return the failed product and all related parts for evaluation (shipping instructions will be provided).
